Current Position: Social Worker, Care Coordination, Lake West Medical Center

Complete psychosocial assessments with hospital patients and their families.
Collaborate with various hospital multi-disciplinary team members to coordinate a comprehensive discharge plan designed to meet the patient’s medical, social and emotional needs. Provide crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on lifestyle. Link patients and families with resources in the community to assist patients with on-going needs and support their health.

Current Position: Therapist at New Directions, Male Residential, Dual Diagnosis Program

I work with male adolescents between the ages of 13 and 18 who meet the criteria for chemical dependence and mental health diagnoses. My role as a therapist is to provide evidenced based, best practice, therapeutic interventions in individual and group settings for our clients and their families. I also provide case management on behalf of our clients to help facilitate and coordinate services.

Current Positions: Community Services Project Coordinator and Program Assistant at Community Partnership on Aging

Both positions, Community Services Project Coordinator and Program Assistant, held at Community Partnership on Aging involve work with the aging populations of at least the five local communities of: South Euclid, Lyndhurst, Mayfield Heights, Mayfield Village, and Highland Heights.

As a Program Assistant, congregate meal and program registration and reservations; nutritional risk assessment; the promotion of educational programs, and provision of information and referrals are priorities. Additional assistant duties include tracking nutrition program data and the completion of the annual nutrition program evaluation for submission to the Western Reserve Area Agency on Aging. 

The role of Community Services Project Coordinator includes: the administration of congregate meal and transportation services funded in part by the Cuyahoga County Health and Human Services Levy and the Department of Senior and Adult Services; a team approach to implementation and maintenance of the agency’s online data base; involvement with the development and implementation of shared medical annual wellness visits in partnership with University Suburban; individual biopsychosocial assessments; Medicare education presentations and individualized appointments; support of social work student internships at the agency, and grant work.

Co-facilitation of an Alzheimer’s Association caregiver support group
