Video Request Form

If you know you will be using a particular video on a specific day, you may place a "video hold" to reserve it for pick-up.

There are a variety of options to request a video hold:

• Call us at (440)-646-8182; or if you are on campus dial ext. 8182.
• Visit us in the Media Center to fill out an orange hold form.
• Fill out the Online Video Hold Request form below.

Please completely fill out the following online form. Search the online catalog to find the specific video you are requesting. Your request will be sent to the Media Center through email. You will need to bring the title and/or call number of the video when y ou pick it up. Note: At this time, online requests are for Ursuline College faculty/staff use only.
