- A completed application to Ursuline College School of Graduate Studies including a $25.00 non-refundable application fee
- Official transcript(s) from the institution(s) which awarded the applicant's baccalaureate degree, and from any institution(s) where undergraduate and/or graduate course work was taken
- A recommended GPA of 3.0 in undergraduate as well as graduate work, based on a 4.0 system
- The Graduate Record Examination (GRE) may be required of students whose undergraduate GPA is less than 3.0
- Three recommendations written by appropriate professionals, (Required recommendation form) one from a professional with whom you worked in a service/ministerial setting
- An interview with a Director of the program
- Successful completion of Foundation Course MM 610, Foundations of Theology
- Commitment to an ongoing ministry
